Minnetonka, MN., October 2, 2007 — The Medica Foundation in it first cycle of funding for 2007 made grants totaling $250,000 for eight projects that address behavioral health issues. An additional $150,000 in funding was granted for five projects that address chronic disease management.
Addressing behavioral health and chronic diseases conditions are important to the Medica Foundation because of their devastating impact and the role they play in a person’s overall health. “The Medica Foundation is committed to funding projects that have the potential to make a significant improvement in the overall health of Minnesota’s population,” said Rob Longendyke, executive director. “These projects do that by addressing conditions that are distressing and often can lead to other types of diseases and complications.”
The projects funded by the Medica Foundation in the area of behavioral health are:
- Achieve!Minneapolis, $50,000 for its Minneapolis Expanded School Mental Health Program. The program provides on-site mental health services for students and families, including crisis intervention, screening, assessment, treatment, consultation and training of school staff, and access to county funded support and treatment services.
- Family & Children's Service, $20,000 for its Violence Risk Screening, Assessment, and Connections Program for High-Risk Children and Families. The grant will help the program increase access to child psychiatric services for children and families identified through professional assessment and diagnosis.
- Fraser, $10,000 for its program, Expanding Day Treatment to North Minneapolis. The program partners with Minneapolis Public Schools to open a new day treatment satellite for preschoolers at the Nellie Stone Johnson School.
- Lifetrack Resources, $30,000 for its Beginnings project. The grant will expand the organization's service capacity through the addition of a Spanish-speaking home visitor with the Beginnings program, a crucial therapeutic and developmental program for highly at-risk children ages 0-3 who have received a mental health diagnosis.
- Mental Health Consumer-Survivor Network of Minnesota, $30,000 for its “Double Trouble:” Peer Recovery Program for Dual Diagnosis. The program provides peer-based support services aimed at reducing the use of chemical elements within the mentally ill populations of Minnesota.
- The Network, $50,000 for it Integrated Behavioral Health Services program. The program promotes the recovery, resiliency and prosperity of high-risk adults through the delivery of five core components in one cohesive package: behavioral health care, health care, housing, employment and community/family reengagement.
- Tasks Unlimited, Inc., $30,000 for its Services to Seniors Program. The grant will be used to design and implement program enhancements for persons age 65 and older with lifelong mental illness, so that they can stay in their residence, maintain their ability to live within their peer group safely and continue their activities to the extent their physical condition permits.
- YouthLink, $30,000 for its Transitional Youth Mental Health Services. The grant will be used to develop an integrated service model providing clinical case management services and group support within the context of adolescent development, targeted at homeless youth (ages 16-21).
The projects funded by the Medica Foundation in the area of chronic disease management are:
- Amherst H. Wilder Foundation, $30,000 for its Chronic Disease Self-Management for Low-Income Seniors program. The grant will be used to offer and expand the availability of the program to 120 low-income and culturally diverse seniors in the east metropolitan area.
- Indian Health Board of Minneapolis, Inc., $30,000 for its Diabetes Self-Management Education program. The grant will be used to hire a registered nurse to work specifically with the underserved populations with diabetes that the Indian Health Board serves.
- Minneapolis Medical Research Foundation, $30,000 for its Asthma Education Tools project. The grant will be used to enhance three existing asthma education and management tools and evaluate them for suitability and acceptance among families and providers.
- Open Cities Health Center, $30,000 for its Community Health Connection Diabetes project. The grant will be used to provide a community health worker to support education for the African American and Hmong communities to promote early detection of diabetes and hypertension and encourage self-management practices for patients and community residents with these chronic diseases.
- Washington County Department of Public Health and Environment, $15,000, for its project: Catch Our Breath! Washington County Asthma Initiative. The project seeks to improve asthma control and chronic disease self management through complete home assessments and intervention and education and resources to patients with asthma and their families.
