Medica offers a portfolio of disease management programs leading to enhanced member well-being and lower health care costs. These clinical programs are designed to provide education and resources that help support the members’ ability to manage chronic conditions. The goal is to improve health, reduce complications through early intervention and monitoring, and to improve satisfaction with health care services.
The disease management programs cover numerous disease states, including co-morbid conditions, as well as 14 rare diseases. Members are stratified based on risk or acuity and receive appropriate education, resources and decision support tools.
As part of this process, Medica has identified members who would benefit from education and support in managing conditions included in our programs. We provide health information specific to a member's condition, and when appropriate, a nurse may work with a member on a regular basis to help manage the condition. The programs strive to member health and satisfaction by giving members a central role in managing care.
