Within FPA you have freedom to choose a primary care physician who will work with you as an active partner to ensure that you and your family receive the personalized health care you expect and deserve. You may also directly access any of the specialty care providers within the FPA Care System, along with other mid-level providers within the offices of an FPA physician such as: physician assistants and nurse practitioners, giving you even greater freedom of choice.
With more than 65 primary care clinic locations, chances are you will find an FPA clinic near your home, work or school. All of our primary care clinics offer access to physicians or nurse consultation 24 hours a day, seven days a week, either in the clinic or by telephone.

FPA primary care and specialty physicians work together to deliver the services you need in a variety of settings: primary care doctor’s office, specialty clinic, hospital, rehabilitation center and other care locations. Your primary care physician may provide your services directly or may assist you to identify an FPA specialty physician to meet your needs.

HealthFront’s Excellence in Quality Awards program recognizes the achievements of Care Systems in providing high-quality care. Our Care System received a 2006 Gold award for excellence in Comprehensive Disease Management - Asthma, Coronary Artery Disease, & Diabetes and the 2006 Silver award for excellence in Comprehensive Disease Management – Preventive Services. We also received the Clinical Innovation Award in 2005 for inclusion of clinical disease management capabilities in our electronic medical record system.
In 2004 we received a Gold Award for excellence in comprehensive disease management for coronary artery disease, diabetes and preventive services and Special Recognition for our program to enhance patient safety in a clinic setting.

FPA offers tailored programs for diabetes care, asthma and coronary artery disease, as well as tools to support tobacco cessation. Programs emphasize patient information and resources to help you reach your health care goals. The diabetes program includes individual sessions with a certified diabetic educator.
FPA’s award-winning Pharmaceutical Care program helps you get the most out of your medications. A specially trained pharmacist meets with you, reviews all your current prescriptions, over-the-counter drugs and herbal supplements to ensure the highest level and most appropriate care possible.
Having safe, secure and reliable electronic medical records is one way FPA leads in providing safer care. Many FPA primary care clinics use or soon will install electronic medical record systems, with many features to help your physician offer safer care. For example, built-in “alarms” help prevent drug interactions; special reminders help ensure your health screenings are current; “best care” alerts ensure you receive the best care available and customized patient education materials help you take a more active role in your care, while built-in links to medical literature assist your physician in providing the most up-to-date care possible.
99 percent of 37,000 people responding to our spring 2007 patient survey stated they would recommend their FPA clinic to their friends. More than 98 percent indicated that they were able to schedule an appointment within a reasonable amount of time, and more than 99 percent rated their overall care as excellent.
When you obtain care from a provider affiliated with your selected Care System, it is considered in-network. That means services covered under your employer’s benefit plan, will be covered at the in-network level—the highest benefit level available through that plan.
Care is considered out-of-network if it is obtained from a provider who is not affiliated with your chosen Care System. Your out-of-pocket costs will be higher for out-of-network services. Depending on the benefit plan offered by your employer, out-of-network services may not be covered or may be covered at a reduced benefit level.
