In an effort to help you find the information you need, we have created resources to help you quickly find providers in your plan's network based on the type of coverage you have. Begin by choosing from the list below.
** Please confirm with the provider's office that they are in your plan's network before your first and each subsequent appointment. If you have questions regarding whether your provider or clinic participates in your health plan, your benefits and/or coverage, please call the Customer Service number on your member ID card.

- Individual and Family if you purchased non-Medicare coverage directly from Medica or an agent.
- Medicare if you are a Medicare member and purchased your coverage directly from Medica or an agent.
- Minnesota Public Programs if you are a Medica member through Minnesota state public programs.
- MCHA if you participate in health coverage offered through the Minnesota Comprehensive Health Association.
Dental Providers (For Dental Only Members)
