Medica DentalComplete^SM

A national dental plan available to Minnesota employers.

With Medica DentalComplete you get:

• No waiting periods.
• Competitively priced plans for groups with as few as two employees.
• Optional orthodontic coverage available with a minimum of five enrolled employees.
• The option of paying all, some or none of the premium costs on behalf of employees.

Medica DentalComplete members may see any licensed provider with no referral. However, choosing a network provider has many advantages, including contracted rates, no paperwork, and no balance billing. The good news is that with more than 40,000 dentists participating in the nationwide network and 1,500 in the local network, seeing a network provider is easy.

Medica DentalExplorer^®

A regional open access plan for non-metro Minnesota and North Dakota employers.

This dental plan is not available for employers located in the seven-county Minneapolis-St. Paul metropolitan area (Hennepin, Ramsey, Carver, Washington, Anoka, Scott and Dakota counties).

With Medica DentalExplorer you get:

• A choice of comprehensive plans for employers with two or more enrollees.
• Orthodontic coverage is available for employers with 50 or more employees.
• 100% coverage of preventive care.
• 50% coverage of other services.
• Annual maximum benefit and deductibles vary by product.
• Freedom to see any licensed dentist.
