    Please note that this formulary could change during the plan year. If we remove drugs from our formulary, add prior authorization, quantity limits and/or step therapy restrictions on a drug, we must notify affected members of the change at least 60 days before the change becomes effective, or at the time the member requests a refill of the drug, at which time the member will receive a 60-day supply of the drug.

*These services are neither offered nor guaranteed under our contract with the Medicare program. In addition, they are not subject to the Medicare appeals process. Any disputes regarding these services may be subject to the Medica grievance process.

Medica Clear SolutionSM is a Medicare Advantage product offered by Medica Health Plans. Medica is a health plan with a Medicare contract. The contract with CMS is renewed annually, so availability of coverage beyond the current contract year is not guaranteed.
