Medica Enrollment Eligibility 2008 Information
- You must be enrolled in Medicare Parts A and B or in Part B only.
- You must continue to pay your Medicare Part B premium.
- You must permanently reside within the Medica Prime Solution service area.
- You must not have End-Stage renal Disease (SRD), unless:
- You are already enrolled in a Medica Plan as a non-Medicare member and you developed ESRD while a Medica member ; or
- You have had a successful kidney transplant and no longer require dialysis; or
- You have medically determined to first have ESRD after the date you elect Medica Prime solution, but before the effective date of coverage under the plan. (The date you elect Medica Prime Solution is the date the enrollment form is signed, the receipt date stamp if no date is on the form, or the date elect is made by alternate means provided by CMS).
- You must be enrolled in Medicare Parts A and B.
- You must continue to pay your Medicare Part B premium.
- You must permanently reside within the Medica Advantage Solution service area.
- You must not have End-Stage renal Disease (SRD), unless:
- You are already enrolled in a Medica Plan as a non-Medicare member and you developed ESRD while a Medica member ; or
- You have had a successful kidney transplant and no longer require dialysis; or
- You have medically determined to first have ESRD after the date you elect Medica Advantage Solution, but before the effective date of coverage under the plan. (The date you elect Medica Advantage Solution is the date the enrollment form is signed, the receipt date stamp if no date is on the form, or the date elect is made by alternate means provided by CMS).
- You must be enrolled in Medicare Parts A and B.
- You must continue to pay your Medicare Part B premium.
- You must permanently reside within the Medica Complete Solution service area.
- You must not have End-Stage renal Disease (SRD), unless:
- You are already enrolled in a Medica Plan as a non-Medicare member and you developed ESRD while a Medica member ; or
- You have had a successful kidney transplant and no longer require dialysis; or
- You have medically determined to first have ESRD after the date you elect Medica Complete solution, but before the effective date of coverage under the plan. (The date you elect Medica Complete Solution is the date the enrollment form is signed, the receipt date stamp if no date is on the form, or the date elect is made by alternate means provided by CMS).
- You must be enrolled in Medicare Parts A and B.
- You must qualify for State of Minnesota assistance in paying for your health care premiums and prescription drugs.
- You must permanently reside within the Medica AccessAbility Solution service area.
- You must not have End-Stage renal Disease (SRD), unless:
- You are already enrolled in a Medica Plan as a non-Medicare member and you developed ESRD while a Medica member ; or
- You have had a successful kidney transplant and no longer require dialysis; or
- You have medically determined to first have ESRD after the date you elect Medica AccessAbility Solution, but before the effective date of coverage under the plan. (The date you elect Medica AccessAbility Solution is the date the enrollment form is signed, the receipt date stamp if no date is on the form, or the date elect is made by alternate means provided by CMS).
- You must be enrolled in Medicare Parts A and B.
- You must continue to pay your Medicare Part B premium.
- You must permanently reside within the Medica Select Solution service area.
**If you have End-Stage Renal Disease (ESRD) and you are in the Original Medicare plan, you may join a Medicare Prescription Drug Plan, but you usually can’t join a Medicare Advantage Plan. If you have had a successful kidney transplant, you may be able to join a Medicare Advantage Plan. Medica Prime Solutionsm is a Medicare Cost product offered by Medica Insurance Company, an insurance company with a Medicare cost contract.
Medica Advantage Solutionsm is a Medicare Advantage product offered by Medica Health Plans and Medicare Health Plans of Wisconsin, [Medica Insurance Company] health maintenance organizations [Insurance Company] with Medicare Advantage contracts. Medicare-eligible beneficiaries must reside within the Medica Advantage Solution service area and be enrolled in Medicare Parts A and B to apply. You must continue to pay your Medicare Part B premium and must not have End-stage renal disease.
Medica Complete Solutionsm is a Medicare Advantage Special Needs Product offered by Medica Health Plans, a health maintenance organizations with Medicare Advantage contracts. Medicare-eligible beneficiaries must reside within the Medica Complete Solution service area and be enrolled in Medicare Parts A and B to apply. You must continue to pay your Medicare Part B premium and must not have End-stage renal disease.
Medica AccessAbility Solutionsm is a Medicare Advantage Special Needs Product offered by Medica Health Plans, a health maintenance organization with Medicare Advantage contract. Medicare-eligible beneficiaries must reside within the Medica AccessAbility Solution service area and be enrolled in Medicare Parts A and B to apply. You must continue to pay your Medicare Part B premium and must not have End-stage renal disease.
