| With UPlan Essential, you select a primary care clinic to provide for your health care needs. The primary care clinic you select determines your designated Care System. The primary care clinic you select determines your designated care system. Primary Care Clinics available for selection will display with your care system name, along with a Primary Care Clinic ID number. Use this number to enroll in the clinic. If no PCC ID number is displayed, it is not valid for selection as a primary care clinc. Please note: If your care system is not listed in the "care system" column, you will need a referral to receive care from this facility. Learn more about care systems.
A care system is a comprehensive network of health care providers who work together to provide health care services. Care systems include primary care physicians and affiliated specialists, hospitals and other health professionals. ** Please confirm with the provider's office that they are in your plan's network before your first and each subsequent appointment. If you have questions regarding whether your provider or clinic participates in your health plan, your benefits and/or coverage, please call the Customer Service number on your member ID card. | |
