Order Plan Materials by Mail

For Minnesota Health Care Programs Members

To request a print copy of your plan materials to be mailed to you, select the items you would like from the list below and Submit the form.

NOTE: Some of these documents are very large (over 1,000 pages) and will not fit in a standard mailbox.
