Are you or your family members living with a serious or chronic health condition? You're not alone. Case managers can partner with you and your providers to help manage health conditions and create a personalized care plan based on your health goals. Then your case manager will provide ongoing coordination and support as you're working toward your health goals.

Open enrollment will take place November 1,  2024 - January 15,  2025. You may qualify for financial help, too, even if you didn’t before.

It’s easy to see how much financial help you can get.  

Your 2024 household income determines if you can get help paying for your 2025 health insurance.


Here are some guidelines for eligibility:

  • Individuals making $20,120 - $58,320 or more**
  • Family of two making $27,213 - $78,880 or more**
  • Family of three making $34,306 - $99,440 or more**
  • Family of four making $41,400 - $120,000 or more**
