To request prior authorization, ask your provider to complete the Transplant Prior Authorization form and fax it to 952-992-3556.

You or your provider can request the help of a case manager. Case managers work in collaboration with you and your provider to manage prior authorization requirement. They will also help you find network facilities for pre- and post-transplant services, such as home health care, home therapy and social service support. To request these services, call 1-866-905-7430.
