Get the care and support you need at no cost with Medica ChoiceCare PMAP. This program is for people with low income who qualify for Medicaid based on income and household size. Medica ChoiceCare PMAP gives you all the support of Medicaid with extra benefits such as a $0 gym Membership.

Ready to enroll?

Apply through MNsure. Once you qualify for Medicaid, the Minnesota Department of Human Services will send you a letter with a form. Tell them you want Medica by filling out the form or calling the phone number provided in the letter.

American Indians can continue or begin to use tribal and Indian Health Services (IHS) clinics. We will not require prior approval or impose any conditions for you to get
services at these clinics. For elders age 65 years and older this includes Elderly Waiver (EW) services accessed through the tribe. If a doctor or other provider in a tribal or IHS
clinic refers you to a provider in our network, you don't need to see your primary care provider prior to the referral.
