This plan provides low or no-cost, high-quality health care coverage to adults age 65 and older who are eligible for Medical Assistance and live in the 50-county service area. It includes all Medical Assistance benefits, as well as Elderly Waiver services and care coordination (if you qualify).

This plan offers a Care Coordinator. These are nurses, social workers or highly trained professionals.
Your Care Coordinator will work one-on-one with you. They’ll find and organize resources and providers to
meet your health care needs and help you maintain a healthy, independent lifestyle.

American Indians can continue or begin to use tribal and Indian Health Services (IHS) clinics. We won't require prior approval or impose any conditions for you to get services at these clinics. For elders age 65 years and older this includes Elderly Waiver (EW) services accessed through the tribe. If a doctor or other provider in a tribal or IHS clinic refers you to a provider in our network, you don't need to see your primary care provider before the referral.
