Your personal health advocate

Health Advocate™ provides you information, assistance, and decision-making tools needed to manage your health and make the most informed health care choices. Your Personal Health Advocate can help you:

  • Resolve health care claim issues
  • Negotiate billing and payment arrangements
  • Find the best doctors, hospitals, dentists 
  • Schedule appointments with providers
  • Estimate costs of specific medical procedures and services
  • Answer your questions about medical test results, treatments or medications recommended by your physician
  • Help you transfer medical records

24-hour registered nurse guidance

Health Advocate Nurseline™ provides 24-hours a day, seven days a week services over the phone. You can consult with a registered nurse to receive guidance on appropriate self-care treatment options for non-urgent illnesses.

Complex case management support

Living with a serious or chronic health condition can be stressful and demanding. To help you cope, nurses are available to help you navigate the health care system, and will work with you to:

  • Keep track of medications, appointments and referrals
  • Communicate more effectively with your doctor
  • Help you understand your health conditions
  • Assess your health needs, develop a plan and follow up on your progress

Additional Wellness Topics

Are you pregnant or considering becoming pregnant? We offer online resources, a list of recommended services and details pregnancy information. You can also sign up to receive tips via text message. Our resources can help you and your baby achieve optimal health throughout your pregnancy and during the first six weeks after delivery.

If you're pregnant or have a baby under age 1, you can receive text messages on your cell phone from Text4baby® to help during this time. These messages include:

When you receive preventive care, you won't be charged for the visit to your doctor. If during the visit you also receive care that is not considered preventive, there may be a charge (or copay). To avoid surprises, it's helpful to know what types of care are considered preventive. Learn which preventive care services are covered under the Affordable Care Act on the U.S. Department of Health & Human Services website. These services are covered 100% when delivered by a network provider.

When you need an organ or bone marrow/stem cell transplant, our transplant program can help you get the care you need in the most effective setting, with the best possible outcome. We’ll provide the information you need to make informed decisions about your care and help you understand your diagnosis and care plan. We can also help you find a Centers of Excellence transplant facility.

Before You Receive Care

Your health care providers must receive prior authorization from Medica before providing transplant-related care (including pre-transplant evaluations). Without prior authorization, these services may not be covered and Medica may not pay your claim.

Prior-authorization is needed for the following transplant types:

  • Bone marrow or stem cell (peripheral or umbilical cord blood)
  • Heart (adult or pediatric)
  • Heart-lung
  • Intestinal
  • Kidney
  • Liver
  • Lung (single or double)
  • Simultaneous pancreas and kidney (SPK)
  • Pancreas after kidney (PAK)
  • Pancreas (pancreas alone)

Request Prior Authorization

To request prior authorization, ask your provider to complete the Transplant Prior Authorization form and fax it to 952-992-3556.

Need More Support?

You or your provider can request the help of a case manager. Case managers work on your behalf to manage prior authorization requirements. They will also help you find network facilities for pre- and post-transplant services, such as home health care, home therapy and social service support.
