Here are your choices:

Check  Keep your plan

You don’t need to take any actions. Your new coverage and monthly premium will start on January 1, 2020. Just make sure to pay your new premium amount by the date on your bill.

  • You will receive your January bill with your new premium amount by late December.
  • If you signed up for automatic monthly payments*, your new premium will withdraw within the first five business days in January.

*If you sign up for automatic payments between December 26 - 31 2019, your automatic payments will not begin until February. You'll need to make a one-time payment for your January premium.

You'll receive new ID cards in the mail before your new coverage starts. Please use your new ID cards starting January 1, 2020.

Change  Change your plan

You MUST do it during Open Enrollment, November 1 through December 15, 2019**. After you’ve determined the new plan you’d like to enroll in and/or the changes needed to your covered dependents, follow the steps below.

**Deadline for Minnesotans is December 23, 2019.

Iowa, Kansas, Missouri, Nebraska, Oklahoma and Wisconsin Residents:

Log into your Marketplace account and follow the steps for changing your coverage. Or, contact the Marketplace Call Center at 1-800-318-2596 (TTY: 1-855-889-4325).

Log into your MNsure account and follow the steps for changing your coverage. Or, contact MNsure’s Call Center at 1-855-366-7873 (TTY: 711).

Complete the Plan Change Form and return to us using one of the following methods. Or, give us a call at the number on the back of your Medica ID card.

Note: If you’d like to make changes to your covered dependents (add or remove) you must complete and return a change form.

Securely fax your form to Medica’s enrollment department at 952-992-2511.

If you prefer, you can mail us the form. Make sure you mail it well in advance.

Cancel  Cancel your plan

You must notify your state’s Health Insurance Marketplace (if you purchased through the Marketplace) or us that you’d like to cancel your coverage by December 31, 2019. Only the subscriber of the policy (primary applicant) can cancel the plan.

You must notify your state's Marketplace (HealthCare.gov/MNsure) you'd like to cancel your plan – Medica cannot cancel your Marketplace plan.

Signed up for automatic payments?

You also must notify us that you would like to cancel your automatic payments. There are two ways to cancel:

  1. Log in to your secure member site. In the Premium Pay section, select Manage automatic payment and follow the steps to cancel payments.
  2. Give us a call at The number on the back of your ID card.

Complete the Policy Termination Form and return to us using one of the following methods. Or, give us a call at the number on the back of your Medica ID card.
