In order for us to release information via email, we must verify your identity so we can provide you with information specific to your health insurance plan as well as document our communications to you. If we are missing any required information, we'll send you an email requesting that information before we can respond to your original question. Once we receive the requested information, we can respond to your request.

Medica is a Cost, HMO-POS and PPO plan with a Medicare contract. Enrollment in Medica depends on contract renewal.
