Continuity of Care

Upon termination of the contract between you and Medica, some members may be eligible for continuity of care if they meet certain medical or cultural requirements.  At the time of termination, Medica may request you provide us with a list of your Medica patients in order to determine if they meet eligibility criteria.

We have laid out the conditions for eligibility for continuity of care as required by either federal law or under each state law within Medica’s scope of coverage. See State-Specific Contract Requirements located under the Supplementary Contracting and Regulatory Requirements section.
