  • This is a broad tiered provider network. Your benefits will vary depending on the provider you visit. It includes:
    • Tier 1 – Preferred Providers
      • 8,290+ Primary and specialty care providers
      • 133+ Hospitals
      • Including the following health care systems and other providers:  Avera Health, Boys Town, Bryan Health, CHI Health, Children's Hospital & Medical Center, Children's Physicians, Genesis Health System, Great River Health System, Mary Lanning Health Care, Mercy Iowa City PHO, Methodist Health System, Nebraska Medicine, UnityPoint Health, University of Iowa
    • Tier 2 – Standard Providers
      • 8,136+ Primary and specialty care providers
      • 142+ Hospitals

Note: Unless it's an emergency, there is no coverage if you visit a provider who's not in the Insure Tiered network. This means you'll be responsible for the full cost of any care.

CHI Health Network

Nebraska Farm Bureau Sales Area - CHI Health

Available in Buffalo, Burt, Butler, Cass, Colfax, Cuming, Dodge, Douglas, Filmore, Hall, Hamilton, Kearney, Johnson, Lancaster, Nance, Nemaha, Nuckolls, Otoe, Pawnee, Saline, Sarpy, Saunders, Seward, Thayer and Washington counties.

  • This is a local care system provider network. It includes:
    • 1,400+ Primary and specialty care providers
    • 30+ Hospitals
    • Including the following health care systems, and other providers: Boys Town, CHI Health, CHI Health Partners and Children's Hospital and Medical Center.

Step 3: Enroll

When you are ready, you must work with your Farm Bureau Financial Services agent to enroll in a Nebraska Farm Bureau Member Health plan.
