Medica Choice Care℠ and Medica MinnesotaCare members:

As of May 1, you will need to change to a new health plan provider.

Medica’s contract with the state of Minnesota to provide Prepaid Medical Assistance (PMAP) and MinnesotaCare plans ends April 30. Here’s what this means to you:

  • If you or someone in your household are in Medica MinnesotaCare or Medica Choice Care, also known as the Pre-Paid Medical Assistance Program (PMAP), you should have received a letter from the Minnesota Department of Human Services (DHS) explaining your options for choosing a new plan. If you have questions about choosing a new plan, please call the Minnesota Health Care Programs (MCHP) Member Help Desk at 1-651-431-2670 or 1-800-657-3739 (toll free). TTY:711
  • This change does not impact anyone in your household who is enrolled with Medica Accessibility® (SNBC), Medica Choice Care MSC+, or Medica DUAL Solution® (MSHO).
  • Medica’s Provide-A-Ride services will be available for you to schedule appointments through April 30. For any appointments beginning in May, you will need to schedule rides with your new provider.
  • Medica Customer Service staff will be available to you after April 30 to help you address any claims issues or service issues that occurred during the time of your Medica coverage.
