Pregnancy Program

Medica's Pregnancy Program is a family-centered model that supports the health of pregnant women and their babies in partnership with the health care provider. Support is provided on a continuum through pregnancy and the post-partum period. Support is focused on improved member understanding of their pregnancy, prenatal care, the needs of their developing baby and preparation for labor and delivery. The goal of the program is to promote healthy outcomes for mother and baby.

Services Provided

  • Case management services that include care coordination, education and support.
  • Assistance in accessing care and identifying Medica participating providers.
  • Referrals to community programs.
  • Educating the member on their anticipated care during the various stages of their pregnancy.
  • Coordinating appointments with an obstetrician and specialists.
  • Arranging transportation assistance.
  • Assisting with community resources to address social determinates of health that may impede desired health outcomes.
  • Assessment of stress levels and markers of depression.
  • Integrated care with behavioral health care, as needed.
  • Arranging for coverage and delivery of durable medical equipment (DME) needed during and after pregnancy.
  • Medication management.
  • Anticipated plan for services and support after the birth of the baby.
