The cost information you’ll find in this look-up tool is an
estimate based on a common pharmacy plan design for Health Savings Account
(HSA) plans. With an HSA, you generally pay the full cost of your prescriptions
until you meet your deductible. The tool will give you a general idea of the
potential costs of various medicines. Your actual pharmacy benefit will
determine your costs. Your plan also may not cover all of the drugs shown in
the look-up tool. This tool includes the HSA preventive drug list.
If your employer is based in Minnesota, North Dakota, South
Dakota, Nebraska, Wisconsin, or Harrison county, Mills county, or Pottawattamie
county in Iowa, and offers fully insured Medica plans, or offers self-insured
Medica plans with the $25 insulin cost relief program, you’ll pay no more than
$25 for insulin per prescription unit. If your plan normally has a deductible
for prescriptions, the deductible is waived for insulin.
