When you need care
VantagePlus has the providers you know and trust from M Health Fairview (the health care organization representing Fairview, University of Minnesota and M Physicians), North Memorial Health and many popular independent clinics, including Boyton Health. While you don't need a referral to see a specialist in the network, it's a good idea to work closely with your primary care doctor to coordinate your health care needs.
How to get the care you need
Not sure what kind of plan you have? Check the back of your Medica ID card. A self-insured plan will state: This plan is administered by Medica Self-Insured. If you don’t see this note, you have a fully insured plan.
Information sharingOne unique feature of an ACO plan is how we work with your provider to coordinate your health care. By sharing member information with each other, Medica and your ACO can help you get the care you need and deliver programs and services to help you get and stay healthy. If you or your dependents don't want us and the ACO to share your information with each other, complete the opt-out form or call Customer Service at 877-252-5564. Please note: You'll still get communications about your care management directly from us and your ACO providers.
Access to primary care providersIn Minnesota, we are required to ensure you have access to primary care providers within 30 miles or minutes of your home, and access to specialists within 60 miles or minutes. Given availability of providers in certain areas, this is not always possible. When that's the case, we apply for a Network Adequacy Waiver from the Minnesota Department of Health.
