How to keep your health coverage
Step 1 - Update your contact information
Get ready to renew by updating your address, phone number, and email. This is important so you can get your renewal information on time.
Call the Member Services number on the back of your Identification (ID) card to update your information.
Step 2 - Watch your mail for renewal info
The Minnesota Department of Human Services (MN DHS) will mail you your renewal notice. Look for an envelope that looks like this:
Inside you’ll find one of these items:
- A renewal form to complete and return, or
- A notice that your insurance has automatically renewed — and you’re all set if the info is correct

Medica DUAL Solution® and Medica AccessAbility Solution® Enhanced are HMO D-SNPs that contract with both Medicare and the Minnesota Medical Assistance (Medicaid) program to provide benefits of both programs to enrollees. Enrollment in Medica DUAL Solution and Medica AccessAbility Solution Enhanced depends on contract renewal.
