Medica Choice Care℠ PMAP provides health care at no cost for people who are:

  • Age 0–64 years old
  • Eligible for Medical Assistance (Medicaid)

Medica MinnesotaCare provides health care with premiums based on income for people who are:

  • Age 0–64 years old
  • Eligible for MinnesotaCare

Medica Choice Care PMAP (Prepaid Medical Assistance Program) and Medica MinnesotaCare are available in the following Minnesota counties:

  •  Becker, Chisago, Clay, Crow Wing, Hennepin, Kittson, Mahnomen, Marshall, Norman, Otter Tail, Pennington, Polk, Red Lake, Roseau, Sherburne, St. Louis, Wadena, Wilkin, or Wright counties.

You need to live in Minnesota and meet income limits and other program requirements to qualify.

8 a.m. – 6 p.m. CT, Monday – Friday

American Indians can continue to use tribal and Indian Health Services (IHS) clinics. We will not require prior approval or impose any conditions for you to get services at these clinics. For elders age 65 years and older this includes Elderly Waiver (EW) services accessed through the tribe. If a doctor or other provider in a tribal or IHS clinic refers you to a provider in our network, we will not require you to see your primary care provider prior to the referral.
