For Medica DUAL Solution® (HMO D-SNP) (MSHO) Members

Over-the-counter allowance

You get an allowance of up to $200 every three months (up to $800 per year) for over-the-counter items that are not covered by Medical Assistance. Order by phone, online, or using the new OTC Health Solutions app available to download from your preferred app store. Get your over-the-counter items shipped direct to your home. The benefit resets each quarter and there's no carryover from quarter to quarter.

Healthy Savings

All DUAL Solution members get no-cost access to the Healthy Savings program. It helps you and your family eat nutritiously and save money on your grocery bill. Shop for promoted items at Healthy Savings network stores and scan your Healthy Savings card or app barcode at checkout.

Our Healthy Food program provides a monthly allowance of up to $150. You can use it on healthy foods like fresh fruits, vegetables, milk, eggs, bread, nutritional drinks, coffee, breakfast foods, pasta, and more.

You can also get one no-cost ride per day to a grocery store in the Healthy Food network.

Reemo™ Smartwatch

Activity tracker

All Dual Solution members qualify for a smartwatch to help track your steps and heart rate. You can view and download your health data. You can give family or a caregiver access to your data, too.

Telemonitoring*

In addition to activity tracking, you may be able to receive a blood pressure cuff and weight scale. They wirelessly connect with your Reemo™ smartwatch to securely track your health data. Reemo will teach you how to set up and use these devices. They'll also set up preferred times to check blood pressure and weight. You can give family or a caregiver access to your information, too.

Reemo will directly call members who qualify for telemonitoring .


Personal Emergency Response System (PERS)

In addition to activity tracking, you may qualify for the Personal Emergency Response feature that can get you access to help in an emergency quickly, 24/7.

Utility assistance allowance

Get a monthly allowance of up to $100 to help pay utility bills. You have two easy payment options: go to any Walmart store and use their MoneyCenter service, or pay online. The benefit resets each month and there's no carryover from month to month.

Get the AbleTo mobile app for on-demand help with stress, anxiety, and depression that uses clinically-provided cognitive behavioral tools. The app provides coping tools, daily mood tracking, guided journeys, and weekly app-based progress check-ins to stay engaged and manage symptoms.

FOODRx program*

Eligible members with certain chronic conditions can receive:

  • Healthy ingredients and recipes
  • Nutrition education
  • Coaching sessions
  • Community referrals to food resources

Representatives from FOODRx will call eligible members to participate in the program.

Memory fitness app

Train your brain with CogniFit! This web-based memory fitness program provides games and training to help assess and improve your memory.

This program is available at no cost to you.

Health education

You'll receive face-to-face and/or telephonic visits over six months with a highly-trained community health worker. The personalized visits focus on your condition-specific health education, self-management, skill development, and more.

You have access to a registered nurse 24/7 by phone. Get guidance on appropriate treatment options and support for non-urgent illnesses. Nurses can also provide simple self-care tips for non-urgent concerns.

Online learning

Enroll online via a secure website for life skills courses on topics like, Practical Math and Safe Online Browsing. Once enrolled, you can access your secure account anytime from a computer or mobile device.

Our case managers can partner with you and your providers to help you manage your physical and mental health. They'll work with you to create a personalized care plan and provide ongoing management and support to help you reach your health goals.

Chronic care management

Our programs can support you with chronic conditions such as asthma, diabetes, or hypertension. You may receive telephonic or online support through our no-cost condition management services. A registered nurse is available to work with you to complete an assessment, develop personal health goals, and provide support and follow-up.

Quitting tobacco

Our Tobacco Cessation Program helps you reach your goal of living tobacco free. The program is customized to best support you and takes into consideration your lifestyle, triggers, and challenges. You'll have access to telephonic coaching as well as nicotine replacement therapy products (patch, gum, or lozenges).

About getting free from tobacco

Routine foot care

Plan members get unlimited routine foot care that includes cutting or removal of corns and calluses, trimming, cutting, clipping, or debriding of nails. You'll also receive hygienic and preventive maintenance care that includes cleaning the feet, soaking the feet, and use of skin creams to maintain skin tone.

Preventive care

One of the easiest ways to avoid illness and stay healthy is preventive care. Make sure all members of your family get the right screenings, preventive services, and immunizations.

Medica DUAL Solution® is an HMO D-SNP that contracts with both Medicare and the Minnesota Medical Assistance (Medicaid) program to provide benefits of both programs to enrollees. Enrollment in Medica DUAL Solution depends on contract renewal.

American Indians can continue to use tribal and Indian Health Services (IHS) clinics. We will not require prior approval or impose any conditions for you to get services at these clinics. For elders age 65 years and older this includes Elderly Waiver (EW) services accessed through the tribe. If a doctor or other provider in a tribal or IHS clinic refers you to a provider in our network, we will not require you to see your primary care provider prior to the referral.
